        Group discussion

        Scenario

        A 65-year-old female patient is admitted to the local hospital because of dizziness, blackout, and vomiting of blood. The provisional diagnosis is “R/O seizures v. gastrointestinal bleeding.” The patient is admitted at 1900 hours (7 PM) to the medical-surgical floor for observation.

        At 2200 hours (10 PM), the patient vomits 100 ml of bright red emesis. Her vital signs are deteriorating. However, the nurses fail to call the attending physician. The patient complains of abdominal tenderness and has abdominal guarding.

        At 0300 hours (3 AM), the patient’s vital signs have worsened, and the physician is called, who orders a stat type and match for blood. (There is a 3- hour delay before the blood is given.) No further communication takes place between the staff and physician.

        When the physician makes rounds, he rushes the patient to the intensive care unit

        (ICU) to stabilise her internal bleeding. She codes in the ICU and expires.

        Who has breached a standard of care? Who can be held liable?
