      SAQ 1: 

      1.       Mr. Sim a 32-year-old male, is diagnosed with haemophilia and was admitted to the medical ward due to anaemia and a low platelet count. The patient complains of feeling very tired and drowsy.

      During health assessment, you noticed red patches on patient’s skin, and you identified as purpura. 

      a) Describe purpura. (2 marks)

      b) Explain EIGHT (8) nursing interventions with rationale to avoid bleeding at the injection or venipuncture site. (8 marks)


      SAQ 2:

      2.          Mr. Yam is a 55-year-old gentleman attending the medical daycare unit for a two-unit red cell transfusion as part of his supportive care for acute anaemia. Fifteen minutes into the first unit he calls the nurse over as he feels unwell. He reports new back pain and feels nauseous. Bedside observations are taken: Respiratory rate 20/min, Temperature 39°C, Heart Rate 100 bpm, Blood Pressure 95/70 mmHg.

       

      (a)    Describe the importance of iron to hemoglobin synthesis, and why is iron deficiency related to anemia? (3 marks)

      (b) Describe your immediate action and investigations of this scenario in the first 15 minutes after this patient’s presentation. (7 marks)
